Košíky is in Uherske Hradiste district, Zlin region, Czechia. Its flag
is at this
website; its CoA at this
page. Flag adopted 22 Nov 2002.
The flag is also depicted in Petr Exner's VexLex 2001 with differences:
1. the middle stripe is yellow
2. the rose has red petals with green separators, not yellow
3. the flag is not vertical (of course)

Košíky, District Uherské Hradište (granted 22th of November, 2001)
Five horizontal stripes alternatively white and blue - (1:1:4:1:1). In
the middle a red rose blossom, yellow buttoned and barbed between two green
bunches of grapes. Grape rachis of both is oriented to the middle of
the field. (got to be translated - grapes are in bar and upper parts
are pointed to the rose, which is in the middle)
